A formal complaint is a document filed by a Complainant, 或由第九条协调员签署, alleging sexual harassment against a Respondent and requesting that the 机构合规办公室 investigate the allegation(s) of sexual harassment. A formal complaint may be filed by the Complainant by:

Requests for appeal may also be emailed to the 第九条协调员 at (电子邮件保护) . Requests for appeal must be received within five (5) business days from the date of determination or dismissal letter and include at least one ground for appeal. -
